Liability Waiver

I understand and give my consent to receive my service(s) from Massage Therapists and Estheticians that are fully licensed Independent Contractors NOT Employees of Lemongrass Day Spa. If I experience any pain or discomfort during the session, I will inform the therapist. Services should not perform under certain medical conditions. I agree it is my responsibility to inform the massage therapist or Esthetician of any health or medical conditions, surgeries, or injuries prior to receiving any service.

All children under 15 must always be accompanied by an adult in this facility.
I also understand that illicit or sexually suggested remarks or advance maid by me will result in immediate termination of service and I will be liable for the full payment of scheduled appointment.
I also understand that THERE SHALL BE NO LIABILITY ON CONTRACTORS NOR LEMONGRASS DAY SPA’S PART SHOULD I FAIL TO DO SO.

DISCLAIMER: If you are receiving any type of massage: deep tissue, cupping, or specialty massage, a possible side effect of slight bruising can be normal side effects. Pregnant women may NOT get a massage until they are at least 16 weeks along. Advanced Esthetics, Chemical Peels Facials & all Waxing Services may cause skin irritation. I am aware of the risks and complications involved with receiving any of these treatments and I release and hold harmless Lemongrass Day Spa from any claims related to services.
